SDM Sticker

1. ID number (manufacture serial number or customer issue serial number)
2. The date of the equipment’s calibration
3. The date when the calibration is due for recalibration
4. Technician who performed the calibration

SDM Certification

• Title of the Calibration Report
• The name and address of the calibration laboratory
• Document number
• Cal date when the calibration was performed
• Name and address of the customer
• Serial number, manufacture, model and type of instrument being calibrated
• The calibration results as found and as left (where applicable)
• The tolerance of the instrument being calibrated
• The calibration procedure used
• Limitation of use approved by the customer (where applicable)
• The master standard used on the calibration with its serial # and last/next cal dates
• Gage status (Pass or Fail)
• Next calibration due date
• Printed name and signature of the technician who performed the calibration
• A traceability statement
• An uncertainty of measurement statement
